Word requests route "/" when saving

I've managed to learn enough about your project to get the test doc to open. The problem I'm having must be related to locking and readonly. Here is what I'm doing:
This opens the file, but it says it is (readonly).

I think I could use some help understanding what word is looking for in order to open the file for editing instead of readonly. Using debug I can see the initial GET is followed by a HEAD and OPTIONS request which are returning OK 200. I'm suspect word couldn't lock the file so it opens as readonly. I suspect this because LOCK is in the SupportedHttpMethods, but doesn't appear to ever be called when opening the file.

Do the GET, HEAD, and OPTIONS requests get the file in edit mode or should there be a LOCK request as well?

I did a trace and found that the visual studio web server (cassini) is returning a 405 "forbidden" on the OPTIONS request even though in debug I see ProcessRequest() returning 200. I added the following to web.config. Some suggested the IIS WebDAV may be causing the problem so this disables it for the site.
      <remove name="WebDAV" />
    <modules runAllManagedModulesForAllRequests="true">
      <remove name="WebDAVModule"/>
Any other ideas on why OPTIONS would result in a 405?
